 Covid –19 Safety Precautions

Office Safety Precautions in Effect During the Pandemic

In accordance with the Indiana State Department of Health and the Centers for Disease Control and Prevention, our office is taking the following precautions to protect our clients and help slow the transmission of COVID-19.

  • On the day of your appointment please check for signs of illness (fever, cough, chills, headache, muscle or body aches, shortness of breath, fatigue, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea). If any of these symptoms are present, please contact us to reschedule your appointment.

  • If anyone is your household has symptoms of COVID-19, please reschedule your appointment.

  • Please wait in the parking lot for your appointment. We will wave you in after we have sterilized and sanitized the office between every appointment.

  • Please wear a mask upon entering the office.

  • All clients will be screened for symptoms and have access to 60% alcohol-based hand sanitizer when in the office.

  • Chairs are at least 8 to 10 feet apart to maintain social distancing during the appointment.

  • The CDC’s designated safety watch and warning levels will be closely monitored and additional measures will be implemented as needed.

Attendance Policy

 

A full session fee is charged for missed appointments or cancellations with less than 24-hour notice. Covid related cancellations are excused from penalty as long as the client calls the office to cancel the appointment before the scheduled arrival time. Clients are expected to pay by cash, check, debit, HCA card, or credit card at the end of each session.

For missed appointments, the client must pay the fee before scheduling another appointment. If the client misses more than one appointment, they will be required to put a credit card number on file and authorize their therapist to charge client’s credit card for any unpaid invoices. Client shall not make any chargebacks to Company’s account or cancel the credit card that is provided as security without prior written consent. Client is responsible for any fees associated with recouping payment on chargebacks and any collection fees associated therewith.
